A Five-Step Guide for Parents to Screen for Scoliosis in Children

Parents can use the following "Five-Step Observation Method" to conduct a preliminary screening for signs of scoliosis. For the most accurate results, it is recommended that the child removes their shirt during the examination.

The 5-Step Scoliosis Screening Process

1. Frontal Observation (Standing)

• Shoulder Level: Check if one shoulder sits higher than the other.

• Arm-to-Body Gap: Observe the space between the arms and the torso; check if the gaps on both sides are symmetrical.

• Pelvic Alignment: Look for any unevenness in the hips or pelvis.

2. Frontal Forward Bend

• Ask the child to stand with feet together and palms pressed together, then bend forward until their back is parallel to the ground.

• Rib Symmetry: From the front, check if the ribs or chest area appear symmetrical, or if one side protrudes more than the other.

3. Posterior Observation (Standing)

• Observe the child from behind to see if the spine appears straight.

• Scapular Alignment: Check if one shoulder blade is more prominent or positioned higher than the other.

4. Posterior Forward Bend (Adam’s Forward Bend Test)

• This is the most critical step. With the child’s back toward you, have them bend forward.

• Rib Hump: Look for an asymmetrical protrusion on one side of the upper or lower back (often referred to as a "razor back"). This is a primary indicator of scoliosis.

5. Lateral Observation (Bending)

• Observe the curve of the back from the side while the child is bent over.

• Check for an abnormal rounding (kyphosis) or any localized humps along the spine.

Supplementary Observations in Daily Life

• Clothing Fit: Notice if the child’s neckline is frequently tilted to one side, or if hemlines and pant legs appear uneven in length.

• Gait Analysis: Watch how the child walks. Look for a noticeable shift in their center of gravity or any signs of limping.

Early detection of scoliosis is key to preventing future complications.
